COVID 19 Waiver of Liability

I understand the contagious nature of COVID-19 and agree to practice social distancing and keep on a face mask at all times while at CSA.

I agree that CSA has put in place reasonable preventative measures to reduce the spread of Coronavirus/COVID-19 and other viruses and illnesses. However, I understand that CSA cannot guarantee that I will not become exposed to or infected with Covid-19.

I understand and agree that this risk may result from the actions, omissions, or negligence of myself and/or others, including, but not limited to, CSA staff and/or clients.

I voluntarily seek services provided by CSA and acknowledge that by doing so I am increasing my risk of exposure to COVID-19 and other viruses and illnesses. 

I acknowledge that I must comply with procedures to reduce the spread or Coronavirus/COVID-19 and other viruses and illnesses at CSA.

I have reviewed CSA’s COVID-19 Protocol and agree to adhere to these action plans. Failure to do so will result in denial of services at CSA.

By signing this document, I agree that if I am exposed or infected by COVID-19 or other infectious disease during my participation in this activity, then I may be found by a court of law to have waived my right to maintain a lawsuit against the parties being released on the basis of any claim for negligence.
